一、現狀與形勢
“十三(san)五”期(qi)間(jian),衛(wei)生健康(kang)行業(ye)大(da)(da)力推進健康(kang)中(zhong)國、數字中(zhong)國兩大(da)(da)戰略(lve)融合落地(di),深(shen)入實施(shi)“十三(san)五”全民健康(kang)信息化發(fa)展規(gui)劃,加快健康(kang)醫(yi)療(liao)大(da)(da)數據規(gui)范應用(yong)和(he)“互聯網+醫(yi)療(liao)健康(kang)”創新發(fa)展,順利完(wan)成各項(xiang)任務(wu),為(wei)支撐(cheng)衛(wei)生健康(kang)事(shi)業(ye)高(gao)質(zhi)量發(fa)展發(fa)揮了重要作用(yong),取得了顯著成效。
(一(yi))制度規(gui)(gui)范(fan)的(de)頂層設(she)計基(ji)本形成(cheng)。出臺《關(guan)(guan)于(yu)促(cu)進和(he)規(gui)(gui)范(fan)健康(kang)(kang)(kang)醫(yi)(yi)療大數據應(ying)(ying)用發(fa)(fa)展(zhan)的(de)指導意見》《關(guan)(guan)于(yu)促(cu)進“互聯網(wang)+醫(yi)(yi)療健康(kang)(kang)(kang)”發(fa)(fa)展(zhan)的(de)意見》,制定(ding)實施“十三(san)五”全(quan)民(min)健康(kang)(kang)(kang)信(xin)(xin)(xin)(xin)息(xi)化(hua)發(fa)(fa)展(zhan)規(gui)(gui)劃(hua)和(he)安(an)全(quan)規(gui)(gui)劃(hua),初步形成(cheng)以信(xin)(xin)(xin)(xin)息(xi)化(hua)建(jian)(jian)設(she)為基(ji)礎、以大數據發(fa)(fa)展(zhan)和(he)“互聯網(wang)+”服務為引領的(de)“一(yi)體兩(liang)翼”發(fa)(fa)展(zhan)格局。印發(fa)(fa)《關(guan)(guan)于(yu)加強全(quan)民(min)健康(kang)(kang)(kang)信(xin)(xin)(xin)(xin)息(xi)標(biao)準(zhun)化(hua)體系建(jian)(jian)設(she)的(de)意見》,制定(ding)實施醫(yi)(yi)院、基(ji)層醫(yi)(yi)療衛生(sheng)機構(gou)和(he)公共(gong)衛生(sheng)信(xin)(xin)(xin)(xin)息(xi)化(hua)建(jian)(jian)設(she)標(biao)準(zhun)與(yu)(yu)規(gui)(gui)范(fan)、省統籌區域全(quan)民(min)健康(kang)(kang)(kang)信(xin)(xin)(xin)(xin)息(xi)平臺和(he)醫(yi)(yi)院信(xin)(xin)(xin)(xin)息(xi)平臺應(ying)(ying)用功(gong)能指引、醫(yi)(yi)院信(xin)(xin)(xin)(xin)息(xi)化(hua)建(jian)(jian)設(she)應(ying)(ying)用技術指引,推進病案(an)首頁書寫規(gui)(gui)范(fan)、疾(ji)病分類與(yu)(yu)代碼、手術操作分類與(yu)(yu)代碼、醫(yi)(yi)學名詞術語“四(si)統一(yi)”,發(fa)(fa)布220多(duo)項衛生(sheng)健康(kang)(kang)(kang)信(xin)(xin)(xin)(xin)息(xi)化(hua)標(biao)準(zhun),逐(zhu)步實現信(xin)(xin)(xin)(xin)息(xi)化(hua)建(jian)(jian)設(she)“書同文(wen)”“車同軌”。
(二(er))互聯(lian)互通(tong)的平(ping)臺(tai)基礎逐步(bu)夯實。國家(jia)全民(min)健(jian)(jian)康(kang)信(xin)(xin)息平(ping)臺(tai)初步(bu)建成,省統籌(chou)區(qu)域(yu)(yu)(yu)全民(min)健(jian)(jian)康(kang)信(xin)(xin)息平(ping)臺(tai)不(bu)斷完善(shan),實現(xian)各級平(ping)臺(tai)聯(lian)通(tong)全覆蓋。建立(li)健(jian)(jian)全全員人口信(xin)(xin)息、居(ju)民(min)電(dian)子健(jian)(jian)康(kang)檔案、電(dian)子病歷(li)和(he)基礎資源等數(shu)據(ju)庫,強化醫(yi)(yi)療服務(wu)、醫(yi)(yi)療保(bao)障、藥(yao)品供應(ying)等應(ying)用系統數(shu)據(ju)集成和(he)業務(wu)協同(tong)(tong)。積(ji)極推(tui)(tui)動公立(li)醫(yi)(yi)院(yuan)逐步(bu)接(jie)入(ru)區(qu)域(yu)(yu)(yu)全民(min)健(jian)(jian)康(kang)信(xin)(xin)息平(ping)臺(tai),依托平(ping)臺(tai)推(tui)(tui)動不(bu)同(tong)(tong)醫(yi)(yi)療機(ji)(ji)構(gou)之(zhi)間診(zhen)療信(xin)(xin)息互通(tong)共享(xiang)。全國建成1700多(duo)家(jia)互聯(lian)網醫(yi)(yi)院(yuan),7000多(duo)家(jia)二(er)級以上公立(li)醫(yi)(yi)院(yuan)接(jie)入(ru)區(qu)域(yu)(yu)(yu)全民(min)健(jian)(jian)康(kang)信(xin)(xin)息平(ping)臺(tai),260多(duo)個城市實現(xian)區(qu)域(yu)(yu)(yu)內醫(yi)(yi)療機(ji)(ji)構(gou)就診(zhen)“一卡(碼(ma))通(tong)”,2200多(duo)家(jia)三級醫(yi)(yi)院(yuan)初步(bu)實現(xian)院(yuan)內互通(tong)。
(三(san))疫情(qing)防(fang)控的應急能力全面(mian)提(ti)升(sheng)。發揮大數(shu)據在(zai)疫情(qing)防(fang)控、監測分析、病(bing)(bing)毒溯源、物資調配(pei)等(deng)方面(mian)的重要作用,搭建跨(kua)部門(men)數(shu)據共(gong)享(xiang)平臺,強化部門(men)協(xie)同(tong)、信(xin)息(xi)聯動、數(shu)據共(gong)享(xiang),支撐重點人(ren)群排(pai)查(cha)與密接人(ren)員追蹤,降(jiang)低社(she)會風(feng)(feng)險(xian)。“互聯網+醫療健(jian)康(kang)(kang)”發揮突破時空限制免接觸優勢,在(zai)保障(zhang)患者就醫需求、降(jiang)低患者感染風(feng)(feng)險(xian)等(deng)方面(mian)發揮了重要作用,開辟了線上抗(kang)疫戰場。運用大數(shu)據追蹤風(feng)(feng)險(xian)人(ren)群,提(ti)高排(pai)查(cha)工作精(jing)準(zhun)(zhun)性、及時性,支撐做到“四早”“四清(qing)”。推(tui)進健(jian)康(kang)(kang)碼政策統一(yi)和(he)標準(zhun)(zhun)一(yi)致,實現核酸檢測結果和(he)新冠病(bing)(bing)毒疫苗接種信(xin)息(xi)全國共(gong)享(xiang),保障(zhang)群眾有序出行,高效(xiao)統籌疫情(qing)防(fang)控和(he)社(she)會經濟(ji)發展(zhan)。
(四)便(bian)民服務(wu)(wu)的應用成效(xiao)不斷(duan)凸顯。推(tui)進業務(wu)(wu)協同(tong)體(ti)系建設,全國(guo)二級及(ji)以上(shang)醫(yi)(yi)(yi)院全面推(tui)進落實(shi)“互(hu)(hu)聯(lian)網(wang)+醫(yi)(yi)(yi)療(liao)健康”10項服務(wu)(wu)30條措(cuo)施,深化便(bian)民惠民“五個(ge)一”服務(wu)(wu)行(xing)動(dong),全國(guo)各級醫(yi)(yi)(yi)院普遍開展互(hu)(hu)聯(lian)網(wang)健康咨詢、分時(shi)段(duan)預約就診(zhen)、診(zhen)間結(jie)(jie)算、醫(yi)(yi)(yi)保聯(lian)網(wang)、檢查檢驗結(jie)(jie)果查詢、移(yi)動(dong)支(zhi)付等線上(shang)服務(wu)(wu),優化改造就醫(yi)(yi)(yi)流程(cheng)(cheng),看(kan)病就醫(yi)(yi)(yi)“三長一短”問題得到有效(xiao)緩解。全國(guo)遠程(cheng)(cheng)醫(yi)(yi)(yi)療(liao)協作網(wang)覆(fu)蓋(gai)地級市(shi)和所(suo)有國(guo)家級貧困縣,實(shi)現(xian)優質醫(yi)(yi)(yi)療(liao)資(zi)源下(xia)沉基層特別是(shi)偏遠農村地區,有力促(cu)進“重心(xin)下(xia)移(yi)、資(zi)源下(xia)沉”。推(tui)動(dong)政務(wu)(wu)服務(wu)(wu)事(shi)項跨(kua)地區遠程(cheng)(cheng)辦理、跨(kua)層級聯(lian)動(dong)辦理、跨(kua)部門協同(tong)辦理,構建便(bian)民服務(wu)(wu)“一張網(wang)”,“互(hu)(hu)聯(lian)網(wang)+政務(wu)(wu)服務(wu)(wu)”效(xiao)能(neng)大幅提升。

總體(ti)來看(kan),“十三五(wu)”期(qi)間我(wo)國全(quan)(quan)民健(jian)康(kang)信(xin)息化(hua)(hua)建(jian)(jian)設(she)(she)成效顯著(zhu),但目前仍(reng)處(chu)在夯(hang)臺壘(lei)基(ji)、爬坡過(guo)坎(kan)的(de)關鍵時期(qi),在基(ji)礎設(she)(she)施、共享應用、投入保障、網絡安全(quan)(quan)等方(fang)面還存(cun)在短板與弱項(xiang),特別(bie)是統(tong)籌協調機制還不健(jian)全(quan)(quan),法(fa)規標準建(jian)(jian)設(she)(she)有待強化(hua)(hua),信(xin)息化(hua)(hua)建(jian)(jian)設(she)(she)投入機制有待完善(shan),專(zhuan)業人才較為(wei)匱(kui)乏,數(shu)(shu)(shu)據要素價值(zhi)潛力(li)尚未充分(fen)激活,“數(shu)(shu)(shu)字鴻溝”“數(shu)(shu)(shu)據壁壘(lei)”依(yi)然存(cun)在,網絡安全(quan)(quan)形(xing)勢嚴峻復(fu)雜,數(shu)(shu)(shu)據治理能力(li)有待進一步提升。從國際上看(kan),全(quan)(quan)球加(jia)速邁進數(shu)(shu)(shu)字化(hua)(hua)發(fa)展(zhan)快車道,特別(bie)是新冠肺炎疫情深刻(ke)沖擊和挑(tiao)戰全(quan)(quan)球醫療衛(wei)生(sheng)(sheng)體(ti)系(xi),數(shu)(shu)(shu)字技術在衛(wei)生(sheng)(sheng)健(jian)康(kang)領域的(de)應用更加(jia)廣泛、影(ying)響更加(jia)深刻(ke)。面對(dui)數(shu)(shu)(shu)字化(hua)(hua)變(bian)革帶來的(de)機遇與挑(tiao)戰,必須進一步夯(hang)實全(quan)(quan)民健(jian)康(kang)信(xin)息化(hua)(hua)新基(ji)建(jian)(jian),培(pei)育(yu)衛(wei)生(sheng)(sheng)健(jian)康(kang)服務新業態,提升衛(wei)生(sheng)(sheng)健(jian)康(kang)行(xing)業發(fa)展(zhan)新動能,構建(jian)(jian)數(shu)(shu)(shu)據要素治理新格(ge)局(ju)(ju),努力(li)實現全(quan)(quan)民健(jian)康(kang)信(xin)息化(hua)(hua)建(jian)(jian)設(she)(she)更高質量、更有效率(lv)、更加(jia)公平、更可持續、更為(wei)安全(quan)(quan)的(de)發(fa)展(zhan)新局(ju)(ju)面。

加強省統籌(chou)區(qu)域(yu)(yu)全(quan)(quan)民(min)(min)健(jian)康(kang)信息(xi)(xi)(xi)(xi)平(ping)(ping)臺(tai)(tai)(tai)建(jian)(jian)設。以建(jian)(jian)立統一的(de)云基(ji)(ji)礎設施為支撐,構(gou)建(jian)(jian)省統籌(chou)區(qu)域(yu)(yu)全(quan)(quan)民(min)(min)健(jian)康(kang)信息(xi)(xi)(xi)(xi)平(ping)(ping)臺(tai)(tai)(tai),支撐省、市、縣(xian)三級(ji)應用(yong),推進一體(ti)化的(de)數(shu)據(ju)采集、匯聚、治理、共享和分析應用(yong)管理。因地制宜(yi)以實體(ti)或虛擬方式建(jian)(jian)立市級(ji)、縣(xian)級(ji)全(quan)(quan)民(min)(min)健(jian)康(kang)信息(xi)(xi)(xi)(xi)平(ping)(ping)臺(tai)(tai)(tai)。以平(ping)(ping)臺(tai)(tai)(tai)為載體(ti)整合業(ye)(ye)務(wu)系統,構(gou)建(jian)(jian)功能一致、融合開放、有機(ji)對接、授(shou)權分管的(de)平(ping)(ping)臺(tai)(tai)(tai)基(ji)(ji)礎功能,逐步實現所有醫(yi)(yi)療衛生機(ji)構(gou)規范接入各級(ji)全(quan)(quan)民(min)(min)健(jian)康(kang)信息(xi)(xi)(xi)(xi)平(ping)(ping)臺(tai)(tai)(tai),縱向聯通上下級(ji)全(quan)(quan)民(min)(min)健(jian)康(kang)信息(xi)(xi)(xi)(xi)平(ping)(ping)臺(tai)(tai)(tai),橫向聯通同級(ji)政府相(xiang)關部門信息(xi)(xi)(xi)(xi)平(ping)(ping)臺(tai)(tai)(tai),暢通部門、區(qu)域(yu)(yu)、行業(ye)(ye)之間(jian)的(de)數(shu)據(ju)交換。探索(suo)推動社(she)會化健(jian)康(kang)醫(yi)(yi)療大數(shu)據(ju)向各級(ji)信息(xi)(xi)(xi)(xi)平(ping)(ping)臺(tai)(tai)(tai)集聚。
構建傳(chuan)染病(bing)監測預警(jing)與(yu)(yu)應(ying)(ying)急(ji)(ji)指揮信息(xi)(xi)平臺。以(yi)(yi)全民健康信息(xi)(xi)平臺為(wei)基礎,按照“整體(ti)(ti)統籌(chou)(chou)、橫向整合、縱向貫通、重(zhong)點(dian)突(tu)出”的原則,建立(li)以(yi)(yi)疾控機構為(wei)支撐,以(yi)(yi)國家級(ji)傳(chuan)染病(bing)監測預警(jing)與(yu)(yu)應(ying)(ying)急(ji)(ji)指揮信息(xi)(xi)平臺為(wei)主體(ti)(ti),各省省統籌(chou)(chou)區(qu)域傳(chuan)染病(bing)監測預警(jing)與(yu)(yu)應(ying)(ying)急(ji)(ji)指揮信息(xi)(xi)平臺共建、共治、共享(xiang),實現(xian)國家與(yu)(yu)省統籌(chou)(chou)區(qu)域兩級(ji)平臺,覆蓋國家、省、市(shi)、縣四級(ji)應(ying)(ying)用(yong)。融(rong)合多源數據,拓展異常(chang)健康事件(jian)、嚴重(zhong)癥(zheng)候群、病(bing)原學檢測、媒(mei)體(ti)(ti)輿情(qing)、社會舉(ju)報等報告渠道(dao),實現(xian)對傳(chuan)染病(bing)疫情(qing)和突(tu)發公(gong)(gong)共衛(wei)(wei)生事件(jian)的快(kuai)速(su)響應(ying)(ying)和高效(xiao)調度處置,為(wei)防(fang)范化解重(zhong)大疫情(qing)和突(tu)發公(gong)(gong)共衛(wei)(wei)生事件(jian)提供(gong)有力(li)支撐。
全(quan)面(mian)推進(jin)醫(yi)(yi)院(yuan)信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)化(hua)建(jian)設提(ti)檔升級。將信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)化(hua)作(zuo)為醫(yi)(yi)院(yuan)基本建(jian)設的優先(xian)領域。按照《全(quan)國醫(yi)(yi)院(yuan)信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)化(hua)建(jian)設標準與規(gui)范》《全(quan)國公共衛生信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)化(hua)建(jian)設標準與規(gui)范》要(yao)求(qiu),二級及以上醫(yi)(yi)院(yuan)持續完(wan)善醫(yi)(yi)院(yuan)信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)平(ping)臺功能,整(zheng)合醫(yi)(yi)院(yuan)內部(bu)信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)系統(tong)。推進(jin)醫(yi)(yi)院(yuan)新一(yi)代數(shu)(shu)據(ju)中心(xin)建(jian)設,實現(xian)醫(yi)(yi)療業務協同一(yi)體化(hua)、惠民醫(yi)(yi)療服務一(yi)站式、精準決(jue)策(ce)支(zhi)(zhi)持一(yi)門戶、信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)資源(yuan)管理(li)一(yi)張圖、數(shu)(shu)據(ju)分析利用一(yi)平(ping)臺、數(shu)(shu)據(ju)資源(yuan)匯聚一(yi)個(ge)庫,強化(hua)對醫(yi)(yi)院(yuan)精細化(hua)運行管理(li)和(he)全(quan)視角決(jue)策(ce)評價的技術支(zhi)(zhi)撐和(he)數(shu)(shu)據(ju)保障。鼓勵醫(yi)(yi)院(yuan)信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)系統(tong)云(yun)上部(bu)署,推進(jin)醫(yi)(yi)學影像數(shu)(shu)據(ju)存儲、互聯網(wang)服務和(he)應用信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)系統(tong)分步上云(yun)。專科醫(yi)(yi)院(yuan)參照相應等級綜合醫(yi)(yi)院(yuan)要(yao)求(qiu),根據(ju)醫(yi)(yi)院(yuan)專科特色和(he)發(fa)展(zhan)需求(qiu)開展(zhan)信(xin)(xin)(xin)息(xi)(xi)(xi)(xi)(xi)化(hua)建(jian)設。

注(zhu)重拓(tuo)寬人(ren)(ren)(ren)才(cai)培養(yang)渠(qu)道,充(chong)分(fen)發(fa)揮(hui)高(gao)(gao)等(deng)院(yuan)(yuan)校、科研(yan)院(yuan)(yuan)所特別是國家健康醫(yi)療大數(shu)據研(yan)究院(yuan)(yuan)等(deng)機(ji)構在全(quan)民健康信(xin)息(xi)化工作中的(de)智力支撐(cheng)(cheng)作用(yong),加快建立(li)適應行業(ye)特點(dian)的(de)新(xin)一(yi)代信(xin)息(xi)技術創新(xin)應用(yong)人(ren)(ren)(ren)才(cai)隊伍培養(yang)體系(xi)。研(yan)究制(zhi)(zhi)(zhi)定(ding)衛生(sheng)健康信(xin)息(xi)化人(ren)(ren)(ren)員配(pei)(pei)備標(biao)準,突(tu)出加強(qiang)數(shu)據分(fen)析(xi)、網(wang)絡安全(quan)等(deng)技術人(ren)(ren)(ren)員配(pei)(pei)備。完善(shan)人(ren)(ren)(ren)才(cai)使用(yong)培訓機(ji)制(zhi)(zhi)(zhi),針對不同層次信(xin)息(xi)化人(ren)(ren)(ren)員的(de)崗(gang)位需求(qiu)制(zhi)(zhi)(zhi)定(ding)系(xi)列培訓計劃(hua)。完善(shan)專家決(jue)策咨詢機(ji)制(zhi)(zhi)(zhi),發(fa)揮(hui)專家在前瞻性研(yan)究、信(xin)息(xi)化規(gui)劃(hua)、重大項目論證(zheng)、新(xin)技術應用(yong)等(deng)領域的(de)智力支撐(cheng)(cheng)。鼓勵(li)各(ge)地探索(suo)建立(li)首(shou)席信(xin)息(xi)官(guan)制(zhi)(zhi)(zhi)度。加強(qiang)對生(sheng)物醫(yi)學工程、醫(yi)學信(xin)息(xi)技術等(deng)新(xin)專業(ye)和交叉(cha)型人(ren)(ren)(ren)才(cai)建設,營造促進(jin)實用(yong)型人(ren)(ren)(ren)才(cai)與(yu)復合型人(ren)(ren)(ren)才(cai)協調發(fa)展的(de)政策環境(jing),為推(tui)動全(quan)民健康信(xin)息(xi)化高(gao)(gao)質量發(fa)展提供有力支撐(cheng)(cheng)。
(四)嚴格(ge)監督(du)評(ping)估,強化任務落實
支持將醫(yi)(yi)院(yuan)信(xin)(xin)息化(hua)(hua)互聯互通情(qing)況(kuang)納(na)入(ru)醫(yi)(yi)院(yuan)績效考核、醫(yi)(yi)院(yuan)等級評審等工(gong)作中,將全民健(jian)康(kang)信(xin)(xin)息化(hua)(hua)建設(she)(she)發展情(qing)況(kuang)納(na)入(ru)衛生健(jian)康(kang)部門(men)的考評范(fan)圍(wei),與(yu)(yu)經費撥付(fu)、設(she)(she)備配(pei)置、績效評價和(he)人員考核相結合(he)。要加(jia)強調查(cha)研究,督促指導信(xin)(xin)息化(hua)(hua)工(gong)程與(yu)(yu)重點(dian)任務的實(shi)施,分析研判風險,及時解(jie)決重點(dian)領(ling)域與(yu)(yu)關鍵環(huan)節存(cun)在(zai)的問題,適時發布年度(du)發展報告(gao),切(qie)實(shi)抓好規劃(hua)落(luo)地落(luo)實(shi)。

堅持安全(quan)(quan)發展、協(xie)同共進的(de)原則(ze),參與(yu)全(quan)(quan)球數(shu)字健(jian)康(kang)國際(ji)合(he)(he)作,加強(qiang)與(yu)多雙邊平臺機(ji)制的(de)對接,深化與(yu)國際(ji)組織、產業聯(lian)盟(meng)和(he)科研機(ji)構的(de)交流合(he)(he)作,推(tui)廣(guang)數(shu)字健(jian)康(kang)相關技術(shu)、產品、標準、服(fu)務(wu)、規則(ze)和(he)共識,注(zhu)重對國際(ji)衛(wei)生(sheng)健(jian)康(kang)信(xin)息化應用(yong)標準的(de)跟蹤、評估和(he)轉化,推(tui)動我國自主技術(shu)與(yu)全(quan)(quan)球同步發展,探索國際(ji)健(jian)康(kang)醫(yi)療發展合(he)(he)作新(xin)模式,不斷提升我國全(quan)(quan)民健(jian)康(kang)信(xin)息化應用(yong)水(shui)平、產業核(he)心競爭力和(he)國際(ji)影響力。
